Ở nhiều người tình trạng nhiễm khuẩn lúc nặng lên lúc thuyên giảm. Hoại tử và sự hủy hoại mô thường được cân bằng bởi chữa lành và [[xơ hóa]].<ref name=Grosset/> Thế chỗ mô bị tác động là sẹo và các khoang chứa đầy vật chất hoại tử phô mai. Cách điều trị lao là dùng kháng sinh để diệt vi khuẩn. == Tiên lượng ==
Tình trạng nhiễm khuẩn tiến triển thành bệnh biểu hiện rõ rệt khi vi khuẩn đánh bại hệ miễn dịch và bắt đầu sinh sôi. Ở lao nguyên phát (khoảng 1–5% ca), điều này xảy ra không lâu sau lần nhiễm khuẩn ban đầu.<ref name=Robbins/> Tuy nhiên đa số trường hợp là nhiễm khuẩn tiềm ẩn kèm theo không có triệu chứng rõ rệt.<ref name=Robbins/> 5–10% ca lao tiềm ẩn phát triển thành lao hoạt tính, thường là nhiều năm sau thời điểm nhiễm khuẩn.<ref name=Pet2005/>

Nguy cơ vi khuẩn tái hoạt động gia tăng ở người có hệ miễn dịch suy yếu do [[ức chế miễn dịch]] hay nhiễm HIV. Người dương tính với lao phổi mà không đồng nhiễm HIV thì sau 5 năm không điều trị, 50-60% trường hợp tử vong còn 20-25% tự khỏi. Tỷ lệ mắc bệnh thay đổi theo độ tuổi. [[Địa vị kinh tế xã hội]] (SES) có ảnh hưởng lớn đến nguy cơ mắc bệnh. Người có SES thấp dễ mắc và chịu hậu quả nghiêm trọng hơn bởi căn bệnh. Các yếu tố nguy cơ dẫn đến bệnh lao như suy dinh dưỡng, ô nhiễm không khí trong nhà, đồng nhiễm HIV, tiếp xúc với môi trường đông đúc, không gian tù túng, thường gặp hơn ở nhóm người này. Sự chăm sóc y tế không thỏa đáng đồng nghĩa người bệnh lao hoạt tính không được chẩn đoán và điều trị kịp thời nên duy trì là nguồn lây nhiễm cho người khác.<ref name=":0" />

=== Dịch tễ địa lý ===
Bệnh lao phân bố không đồng đều trên toàn cầu với khoảng 80% dân số nhiều nước châu Phi, Caribe, Nam Á, Đông Âu xét nghiệm tuberculin dương tính trong khi tỷ lệ này đối với dân số Hoa Kỳ chỉ là 5–10%.<ref name="Robbins" /> Tại các nước phát triển, bệnh lao ít phổ biến hơn và chủ yếu được thấy ở những vùng nông thôn. Tại châu Âu, tỷ lệ tử vong vì bệnh lao đã giảm từ 500 trên 100.000 người năm 1850 xuống 50 trên 100.000 người năm 1950.

Những tiến bộ trong y tế cộng đồng đã giúp đẩy lùi căn bệnh trước cả khi có kháng sinh, dù vậy nó vẫn là một hiểm họa không thể xem thường. Phải đến kỷ nguyên kháng sinh con người mới có thể yên tâm về một phép trị bệnh hiệu quả. Tuy nhiên, hy vọng khống chế hoàn toàn căn bệnh đã dần tiêu tan bởi một số yếu tố bao gồm khó khăn trong việc phát triển một vắc-xin hữu hiệu, quá trình chẩn đoán tốn thời gian và tiền bạc, thời gian điều trị kéo dài, lao liên hệ HIV gia tăng, và sự xuất hiện của những trường hợp kháng thuốc vào thập niên 1980.<ref name="Lancet11" /> 

Vào năm 2010 tỷ lệ mắc bệnh trên 100.000 người ở những khu vực khác nhau trên thế giới là: toàn cầu 178, châu Phi 332, châu Mỹ 36, Đông Địa Trung Hải 173, châu Âu 63, Đông Nam Á 278, và Tây Thái Bình Dương 139.<ref name="WHO2011Control" />

Trước [[Cách mạng Công nghiệp]], dân gian thường liên hệ bệnh lao với [[ma cà rồng]]. Khi một thành viên gia đình qua đời bởi căn bệnh, những thành viên nhiễm bệnh khác sẽ dần dần suy nhược. [[Albert Calmette]] và [[Camille Guérin]] đạt thành công thực sự đầu tiên trong chủng ngừa bệnh lao vào năm 1906 với việc sử dụng chủng vi khuẩn ở bò bị làm yếu. Hồi thế kỷ 19 và đầu thế kỷ 20, nỗi lo bệnh lao lan rộng trong cộng đồng khi căn bệnh phổ biến ở nhóm dân cư nghèo đô thị. Cứ một trong bốn người tử vong ở Anh vào năm 1815 thì là do bệnh lao. Tỷ lệ này ở Pháp năm 1918 là một trên sáu. Sau khi được xác minh có tính lây nhiễm vào thập niên 1880, lao được xếp vào nhóm bệnh phải khai báo ở Anh.
